St Luke’s Home Care
At St Luke’s Care, our mission is to provide “quality care with a personal touch”. Our Home Care services are no exception.
Making decisions about care can be difficult for everyone involved. St Luke’s Care understands this and will be with you every step of the way.
At the centre of every care plan is an individual, with unique experiences and needs. We provide a range of services to ensure that these needs are met with the best possible solutions. Home Care is not a ‘one size fits all’ scenario. How much or how little care is received is entirely determined by the individual, and can easily shift to adapt to changing care needs.
With St Luke’s Home Care, your personal Case Manager will help you select which services you do and don’t need to create a care package that is tailored to you.

What Home Care options are available?
St Luke’s Home Care provides three streams of services which can all be tailored to suit the individual:
Home Care Packages are government subsidised. To receive a package, you need to contact My Aged Care and arrange for an Aged Care Assessment Team (ACAT) to come to your home and assess your needs. They will then decide what level of care you are eligible to receive. Once you have been allocated a package, you can then choose a provider.
Private Home Care allows people to access Home Care without referrals or assessments. Self-funded Private Home Care provides a wide range of services that can be used in isolation or to ‘top up’ a government subsidised package.
Being a carer can be immensely rewarding, however, it can also be physically and emotionally draining. Respite for Carers allows carers to take time for themselves, attend appointments and meet other commitments. Government subsidies are available by contacting My Aged Care.
