Home Care PackagesExtra help for independent living at home

For many, staying in the security and comfort of home and retaining a fully independent lifestyle is an excellent option, especially with the right care and support on hand.

St Luke's Home Care offers a range of packages designed to help elderly people live independently in their own home with extra care as required.

Community Aged Care Packages (CACP)

These individually tailored and flexible services packages can include:

  • Assistance with everyday showering, bathing and toiletry requirements
  • Help with dressing, undressing and grooming in the morning and/or evenings
  • Light housekeeping to help maintain a clean and tidy home
  • Guidance on mobility equipment, transfers and hire of appropriate equipment
  • Company and support on short outings, shopping trips and coffees
  • Facilitation of regular communication between family, friends and medical professionals
  • Monitoring regular medications to help avoid stress and errors

Extended Aged Care at Home Packages (EACH)

We also offer individually designed and coordinated programs for those with complex needs who are eligible for nursing care, but choose to stay at home. If the requirements become too difficult, or the person can no longer be safely cared for at home, it may be necessary to refer them to an appropriate care provider or residential aged care facility.

CACP and EACH are funded through Commonwealth Aged Care Programs with modest fees paid directly by the client. For some services a full fee may apply. To be eligible for funding an obligation free assessment by an Aged Care Assessment Team (ACAT) is required. Click here to find the ACAT location nearest you.
