For many, staying in the security and comfort of home and retaining a fully independent lifestyle is an excellent option, especially with the right care and support on hand.

St Luke's Home Care offers a range of packages designed to help elderly people live independently in their own home with extra care as required.

Community Aged Care Packages (CACP)

These individually tailored and flexible services packages can include:

• Assistance with everyday showering and bathing requirements
• Help with dressing, undressing and grooming in the morning
• Light housekeeping to help maintain a clean and tidy home
• Guidance on mobility equipment, transfers and hire of appropriate equipment
• Company and support on community access outings and shopping trips
• Monitoring regular medications to help avoid stress and errors

Extended Aged Care at Home Packages (EACH)

We also offer individually designed and coordinated programs for those assessed with complex needs who are eligible for home level of nursing care, but choose to stay at home and can be appropriately managed safely in the community.

Extended Aged Care at Home Dementia Packages (EACHD)

These packages are available for older persons who are assessed as needing high level of care, presenting with dementia and/or challenging behaviour, but are able to be managed safely in their own home.
